The Alzheimer’s Disease Assessment Scale (ADAS) is the most often used scale to measure change in many international multicenter trials, and epidemiological studies. Use across countries assumes straight translation and adaptation of the scale. Appropriate translation requires that the scale also be adapted to cultural differences, leaving it otherwise and psychometric properties unaffected. These meaningful problems have not yet received accurate attention. The aim to EURO-HARPID is to design transnational and psychometrically appropriate versions of widely used instruments such as ADAS, in most Western European Language. International cooperative study in eight European Western countries (Belgium, Germany, France, Netherlands, Italy, Spain, Sweden and UK). 1) Inventory of similarities and differences between the versions already used in participating countries. 2) On the basis of this, national versions of the ADAS were adapted in order to meet comparable psychometric criteria. In each country, 40 patients with Alzheimer's Disease were examined. 1) The inventory shows substantial differences between the ADAS’s versions used in participating countries, and indicate that the issue of translation and harmonization has not been adequately dealt with so far. 2) Validity across countries of adapted versions of the ADAS appeared satisfactory, although minor differences still existed. 1) There is a need for harmonized versions of instruments for dementia that can be used in transnational studies. 2) Harmonized versions will be available in 2000.
