Advances in clinical science and operational practices are allowing patients with chronic conditions to live longer and healthier lives. However, this progress comes with a dilemma - chronic health conditions increasingly drive health care resource use and underlie much of the rising cost of health care. Kaiser Permanente (KP) has implemented comprehensive care and disease management programs to guide and support clinicians and patients. These programs build on and leverage Kaiser Permanente foundation of integrated organizational oversight and decision making involving the payer, delivery system and clinicians. Care delivery is based on evidence based medicine and the Chronic Care Model, actively combining teams of people with health technologies to efficiently deliver the right care at the right time to the right person. The overarching goal is to “make the right things easier” for patient and clinician alike.

Ce colloque sera l’occasion d’inviter un conférencier de marque dans le domaine de la collaboration interdisciplinaire en soins de première ligne. Parmi les personnes ciblées, mentionnons le Dr Paul Wallace du Kaiser Permanente ainsi que les Drs Bodenheimer, Wagner ou Grumbach qui ont élaboré le Chronic Care Model. Des démarches sont en cours auprès de différentes compagnies pharmaceutiques pour trouver les fonds nécessaires pour couvrir les dépenses de ce conférencier invité.
